Question: I am a 75 year old black female. I had a massive heart attack andI have 2 stents and have been in heart failure for 9 years. Two years ago I have experienced symptoms that no physician has been able to diagnose. At the top back between my shoulders I have a burning sensation. It swells and turns red. Feels like pins and needles shortly after the burning sensation starts then tiny bumps form. WHEN I am in this state my skin hurts when fabric rubs against it. Even labels hurt when they touch my skin. The area around my neck has turned deeply irritated with a raw feeling. Whatever it is I experience some remission for about 4-6 months. Then it flares and becomes active. These same symptoms are repeated in my lower back. My rheumatoid factor is high 2 physicians have said that I have Rheumatoid Arthritis and 2 have said I do not have Rheumatoid. I do have Osteoarthritis. Often I have a feeling that something is crawling over me. Because I am on Warfarin I can not take NSAIDs for my pain. Both of my hands are swollen and painful. After careful review it seems that it's actually a multitude of factors the are affecting your health and causing the symptoms you have right now.

On top of the list falls Fibromyalgia that is a strange but very important possibility your case. Your symptoms of burning sensations, swelling, redness, pins and needles, tiny bumps etc all warrant you to get evaluated for Fibromyalgia. In Fibromyalgia patients have same symptoms like: Numbness, weakness, pain soft tissues especially in areas of shoulders and neck (upper mid-trapezius points), burning sensations, sensations of creeping/crawling over body etc.

Coexistence of Osteoarthritis (a non-inflammatory rheumatic disease) is well documented in patients with Fibromyalgia. And, you also are suffering from Osteoarthritis.

Warfarin also may be adding up to your symptoms of Paresthesias (changed sensations on skin and usually taken as feelings of pain and burning, pins and needles). Other side effects of Warfarin include skin rash and deranged blood supply to skin tissues. small blood vessels that supply skin and Small nerve endings in skin are affected by Warfarin. Results can be dry, irritated and painful skin with deranged normal sensations.

Autoimmune disorders also may contribute to some of the symptoms related to skin.

Other options may include local irritation/allergy due to cloth tags, specific fabrics etc. You need to rule it out if this is seasonal looking disease like you said for 4-6 months it is gone totally. May be that symptom free season's clothing doesn't irritate your skin.


Please consult your Physician for physical examination and to get Fibromyalgia ruled out. Also ask him to test for Vit. D and Vit B12 deficiency.
