Brief Answer:
Constipation is the likely cause which needs to be corrected

Detailed Answer:
Thanks for asking on HealthcareMagic.

The first question that I would need to ask is that whether he has constipation. If so:
1. He would need to solve this problem first. He would need to take some stool softener like Syrup Lactulose along with plenty of green leafy vegetables.
2. Sitz bath should be helpful. He would have to take water as hot as skin can take and has to sit on it with his anal opening dipped in the water. This has to be done for around 20 mins twice a day.
3. Annovate cream may be applied. Vitamin E lotion can also be used.
4. Avoid red meat or spicy food. Light diet preferrable.

Hope that helps.

In case you found my answer to be helpful, I would be glad if you leave a 5 star rating and a positive review.
