Brief Answer:
Rule out the possibility of anal fistula

Detailed Answer:
Hi
Welcome to HCM.
I have gone through the pictures.
Rather than rash it looks more like red, swollen area, indicating active infection. Examination is required to rule out the presence of any fistula.

There are some small boils are also seen, might be due to friction caused by the hair, but from the pictures I feel it is less likely to be wart.

I would suggest you to see your general surgeon, for examination and further help. A course of antibiotics along with local management is required and can be decided on examination.

Hope this helps.
Take care.
