Question: I have a five year old with pink, raw and peeling skin inside the buttocks area. It's been there on and off for about five weeks and he says "mom my butts on fire." His pedi thinks it's from constipation and ripping of skin, but it's more peeling than ripping. I feel like it's a fungal infection but he's not complaining of itching. What else could it be? He is a terrible eater; drinks a lot of lemonade and has a mild egg allergy (.34) and a mod sesame allergy (1.00).

There is a possibility of fungal infection.

I treat such cases with clotrimazole powder to be applied twice daily for 15 days.
It is an antifungal, safe and effective in children.

The area bleeds a little after a bowel movement and he still says its on fire. Its pink and raw and peeling. Could it be a strep infection? Can it be swabbed for a fungal infection?

Get swab done and also start treatment.

Thanks for the photograph.
It is looking like a mixed bacterial and fungal infection.
Swabbing for fungal as well as bacterial growth is important.
It's not looking like streptococcal infection else it would have spread faster.
Start applying mupirocine ointment in night and clotrimazole powder in day.
This will take care of infections as well as keep the area dry.
Give him tablet albendazole 400 mg single dose.
This is important to take care of any intestinal worm infestation.
The above mentioned medications should be available over the counter.
