Question: Swelling and intermittent bleeding from anus for years. Usually painless. Last few days has flared up and is very sore and bleeds when going to toilet. Please advise

Hi.
Thanks for your query and picture.
Read and understood that you have swelling and intermittent bleeding from anus for years. This is usually painless but is sore and and bleeds when going to toilet. Has flared up in last few days.

The picture shows most probably that there are 2 prolapsed piles (hemorrhoids) with one showing granulomatous transformation, hence the bleed.
I would advise you the following:
Get clinical evaluation, per-rectal and proctoscopy examination to decide the most probable cause.
And get operated at an appropriate time.

This helps in a few ways. You get rid of the disease and you get a perfect his to pathological diagnosis.

Thanks. Presume it doesn't look like anal carcinoma then? I was very constipated around 8 years ago and felt a tear and could see what looked like a fissure. The problems seem to come from this area still. Thought it was a sentinel pile and hypertrophied papilla...it's very much on outside and can feel a hole behind..

8 years is a long time to say this to be a carcinoma. And this is always a histopath diagnosis. No one can have such a long history.
As you have rightly mentioned, it usually starts as constipation to have anal fissure with a sentinel pile and progress to the present position.

I would advise in such a situation the following:
Sitz bath at least twice daily.
Proper laxative and dosages to have soft stools.
Antibiotic and metronidazole coverage as there would be an obvious injury.
Antiinflammatory medicines for reducing swelling and pain if you have.
Further investigations and proper treatment under the care of a General Surgeon.
