Posted on Mon, 22 Jun 2015
Question: Hello, I am suffering with rectal pain and itching. The pain only occurs after a large/hard bowel movement and can last for a couple of days. I can feel a cut on my anus which bleeds occasionally, usually after a large or hard bowel movement. I believe I have a chronic anal fissure. What treatment do you recommend?

This has been bothering me now for almost 3 months however I am yet to use any creams as I was hoping it would heal naturally but I have decided enough is enough.

I am 21 have no allergies and I just want to sort this out so I can move on with my life!

Brief Answer:
I recommend Sitz baths, good diet, fruits, vegetables and exercise!

Detailed Answer:
Hi and thank you so much for this query.

I am so sorry to hear about this anal pain and itch that you have reported. I have reviewed your information fully and there are a couple of things you need to do. Like you rightly suspected, there is probably a chronic fissure in this area. The most common cause of this is constipation, hard stools and excessive efforts when defecating.

1. You need to eat health, much fruits, vegetables fibers and regular exercise. These activities would ensure that the stools are soft and make for easy defecation. Each time the stool is strong and you strain while defecating, the chances of sustaining a tear are high. This is what has been happening. Whenever you feel like going for bowel movement, please make sure you do not postpone doing so.

2. For now, do Sitz baths to help heal the probable fissure. This is done by adding iodine to a bowl congaing 1-2liters of lukewarm water. Sit inside at least thrice a day and after every bowel movement.

3. If these measures prove difficult, go on and try a pill for constipation. You could use metamucil, miralax or lactulose at doses recommended by the pharmacist. This, only as the last option.

The most important thing, maintain sot stool. Healthy diet, fiber, fruits, vegetables, regular exercise and drinking much water are the best ways to tackle this. If the pain is severe and you cannot tolerate, please take Ibuprofen for this as well.

I hope this helps. I wish you well. Thanks for using our services and do feel free to ask for more information and clarifications if need be.

Thank you for your reply!

Sitz baths are not an option at the moment due to my work commitments abroad.

I will make the necessary changes to my diet but I would like to know about a topical cream to apply.

Would either of these be of any benefit

Glycerin Trinate?
Zinc Oxide Cream?

Or a hemorrhoid cream?

Many Thanks!

Brief Answer:
I will go with Zinc oxide cream for now!

Detailed Answer:
Hi and thanks for this follow up precision.

I appreciate your honesty and sincerity. I will recommend zinc oxide for now. It is good at keeping the area dry and would also help with healing. Also, try using wet wipes after visiting the toilet to cleanse thoroughly after defecating.

Keep me updated with your response too treatment.

Until then, I wish you a speedy recovery.
