-Since how long do you get these fluid filled bumps/pus filled bumps?
-What are the symptoms associated?Pain/Burning etc?
-Is hyperhidrosis (increased sweating of the palms and soles) also associated?

Brief Answer:
Possibility of Epidermolysis Bullosa Simplex

Detailed Answer:
Hi.

I would like to know the duration of blistering? Is it long standing since childhood? If Yes, I would like to keep a possibility of Epidermolysis Bullosa Simplex (Weber Cockyane type).

Epidermolysis Bullosa Simples(EBS) is a collection of disorders of keratin that produces intra-epidermal blistering with little internal involvement. Lesions usually heal without scarring.

Mild EBS of the Weber-Cockayne subtype is the most common type. Blisters are usually caused by an obvious traumatic event. They can be mild to severe and most often occur on the palms and soles. Hyperhidrosis may also occur.

Skin biopsy is required to make the diagnosis.

There is not yet any specific cure for the disease but attention must be paid to facilitating wound healing. Prevention of trauma to the skin reduces blistering.

Wound care is important. Do not let crusts and fluids build up, as they facilitate infection. Topical antibiotics may be required.

Brief Answer:
Get a confirmatory diagnosis

Detailed Answer:
Hi.

I can understand you concern for some specific remedy, apart from salt water soaks. But, first and foremost, you need to get a skin biopsy for a confirmatory diagnosis.

Your history is suggestive of Epidermolysis Bullosa Simplex(Weber Cockayne type).

Prevention of trauma is the key and the only preventive measure in EBS.

For blisters that have already formed, you could use a topical OTC antibacterial ointment e.g 2% mupirocin ointment twice daily over the blisters, for prevention of secondary bacterial infection. An Oral antibiotic e.g penicillin group of antibiotics may also be required if the lesions get secondarily infected.

I would again stress on the need for a confirmatory skin biopsy.

Brief Answer:
Treatment options in EBS

Detailed Answer:
Hi.

The increase in blister development that occurs during hot summer months is thought to be attributable to an increase in the coefficient of friction of the skin (frictional trauma) caused by sweating and raised skin temperature.

Treatment options include:

-20% Aluminium chloride compresses, apart from topical antibiotics.

-Oral Tetracyclines e.g Doxycycline 100mg twice daily

-Botulinum toxin A is a newer treatment modality, which considered to be simple, safe and effective.

-Prevention of trauma and prevention of bacterial infection of blisters remains the foremost and most important measure.
