What Causes Premature Ventricular Contractions After Smoking Meth?

Question: smoked meth last night. (about 2hrs ago was last time) Have taken 1mg of Xanax and 100mg of Mettoprolol (about half hour ago). Have sharp pain on my heart. I suffer from PVC (apparently benign) I also have tambocor. Should I take this also? Not sure what to do - This is urgent. I live in a place where there is no ER close by. My Bp is 135/95 and my heart rate is 65 beats per minute but irregular

Premature ventricular contractions can occur in a healthy person of any age, but are more prevalent in the elderly and in men. It could be due to multiple reasons like adrenaline excess, drugs like cocaine, stimulant, smoking etc. A PVC may be perceived as a skipped beat or felt as palpitations in the chest and leads to irregular heart rate. In your case, it may be due to meth.

Best treatment is eliminate the trigger and for now tambocor is effective drug for PVC, so you can take it. There is nothing harm in taking it. It would subside pain due to PVC.

thanks doctor Kapoor- can I take it on top of the metoprolol I took? Also - Just checked again and my hear rate is up to 99 and BP is 121/91

Your vitals are stable, but chest pain is due to PVC. Tambocor would control PVC. There is nothing harm in taking it. You have taken metoprolol 100 mg half an hour before. You can wait for half an hour more, if it does not subside than better to take lesser dose of tambocor about 25mg.

Dr. The heart arrhythmia has not improved - I have been reading online (I know this is not the wisest thing for a lay person) - but they site said that Tambocor may increase arrythmias to the point of being fatal and NOT to mix with Metoprolol. Any advise? I took the 25mg earlier (3 hrs ago). Shal I just take another Metropolol?

I note your specialty is psychiatry. Is there a cardiologist you can refer my inquiry to?

Sorry for delayed response. I think, now would be ok, but remember tambocor is a anti-arrhythmic drug and it has good response on PVC. I could increase, but it has good response on arrythmias. You can take both drug together in acute condition. There is no harm in it. In your case, PVC is due to meth and i would stay, till effect of meth would stay on heart. Complete effect of meth come down in 5 to 12 hours.

If you want to take cardiologist help, you can take.

Hi

The basic behind this combination is to maintain heart rhythm. Flecainide is anti-arrhythmic drug and metoprolol is beta -blocker. Both in combination can be used to maintain blood pressure and cardiac function.

You had taken 100mg metoprolol and 25 mg flecainide. At this dose, chances of getting addiction effect is less.
