Brief Answer:
Following causes to swelling of ankles where injury is excluded...

Detailed Answer:
Hi,

I understand your concern.

Let me recapitalize your case:
-69 years old male
- symptoms: swelling of ankles . Can you send a photo to me?
- causes: excluded injury

You say to have already done echocardiogram and venous Doppler of the lower extremities but now report attached or explained. Can you please give the results of these examinations? They are important in excluding causes related to heart diseases and deep vein thrombosis of lower extremities.

Furthermore, can you please tell if you are having problems with liver? kidneys? Any test run to check them out? If yes, please provide the reports.

Causes will vary from local (deep vein thrombosis and injury - that is excluded) to systemic (related to heart/ liver/ kidney).
Arthritis of the ankle is also to consider and rule out by X-ray of the ankle and arthritis blood tests.

Looking forward to have the requested photo and info in.
