I read your question and i understand your concern. Low grade fever with symptoms of fatigue, insomnia and musculoskeletal origin needs work up to rule out the cause as there are thousands of possibilities that can cause low grade fever.

You will need work up in effort for finding a focus of infection and will need baseline investigations like blood picture, blood and and urine culture and sensitivity, chest radiographs, liver and renal functions, ESR, CRP and other inflammatory markers and last but not the least a detailed history and physical examination guided clinical correlation to sort out the cause.
