I can understand your concerns

Since it is also painful while passing stool it suggests that there is anal fissure along with hemorrhoids

For this I will suggest to you the following which I found very useful in my patients suffering from the same condition

Make your stool soft by taking lots of fibres and fluid in your diet. Add psyllium fibres. take three teaspoon in glass of water at night. I found Fabolite very useful in my patients as it dissolves completely and taste good. Titrate its dose according to the response.

Take tablet calcium dobesilate thrice a day, comes by name of dobesil for 2 weeks

For pain do sitz bath and continue with smuth ointment. And apply it before and after defecation

For farts avoid dairy products, pea, cabbage and pulses. take capsule simethicone thrice a day, brand name medigas

All the above mentioned drugs are available OTC
