Question: Since this morning innoticed small tiny painful and itchy boils on the basis of my peni, what it could be? And what do i use? I have applied twice Neosporin

Dear Doctor,

Please check ghe attached photo, i have little pain now, I took a course of Antibiotic called Augmentin 625 mg, now these small boils have dried up and all the clear liquid has come out, there is some redness in that base area little pain when I press it, do I put any special cream? I did you two types of cream one callsd Tropical cream and Input few days Neosporin as well. Please let me know if I need to visit a Dermatologist?

Thanka very much for your prompt reply, i am arraching two pictures again, yes i am sexually active only with one partner but i did have a unprotected sex with another partner that was on the March 2nd and my symtomps of boils just happened last week while I was at Gym I felt pain and itching and when i came home i noticed three or four small boiks on the base of my Peni, they were painful, I put Neospirin on that and the boils have gone and i have this redness and the area is sore and i have little pain, since the March 2nd I have not had sex !

I suggest a screening blood test for genital herpes

Hi. Thank you. I have reviewed the Image. I recieved only one Image. I can see an eroded area with discharge and crusting.

I would keep a possibility of either genital herpes Or bacterial skin infection. I would particularly like to rule out possibility of genital herpes.
Topically you may continue with just neosporin ointment, twice daily.
You must take a screening blood test for herpes i.e Elisa for Hsv 1 and 2, (IgG and IgM). In case the ulcers are still active a PCR for Hsv DNA would be highty sensitive and specfic as well as confirmatory. It is a swab test.

Dear Dr. Kakkar,

Thanks for your prompt reply, since this morning I am noticing again some new boils with liquid inside and they are painful, please check the attached photo which i just took it. Please guide me what to to do? Seems like the ulcer is still active, should I go Monday morning see my primary care physician? As I am very much concern about this problem. Should I put any other oinment in the meantime? Or should I take any other oral medication? I will give the blood on Monday morning testing genital herps or any other condition which I might have?

This looks very much like genital herpes.
I would suggest an Oral antiviral e.g either valacyclovir or acyclovir for a week. The lesions should heal within a week on antivirals. Topically you may use an antibacterial ointment e.g either Mupirocin Or Neosproin Ointment, twice daily.
Oral antivirals are prescription medications and I suggest that you talk to your primary care physician for the needful.
You may also go for a DNA PCR for HSV for a confirmatory diagnosis of genital herpes Or a screening blood test for Hsv [ELISA for HSV type 1 and 2 (IgG and IgM)].

Dear Dr. Kakkar,

Thanks for your detailed reply, I have given a blood work ans test should come within four days, I saw my Primary care physion today and he prescribed following medications Acyclovir oinment usp 5% and Valacyclovir 1GM tablets for 30 days and 2 more 30 days prescription to follow, after seeing the photos he also said it is genital herps, my question is to you is after I am healed completely in next 60 to 90 days, would I be able to have unprotected sex with my partner? As we always had unprotected sex, please confirm or how long should I wait? Or do i need to give another blood test after 60 days to see if the virus is completely gone? Is this virus treatable? Or once you have it? It lasts life time? Please guide. A Kind regards Ton

HSV stays inside the body for ever; there might be future recurrences

Detailed Answer:
Hi.

For an episode of genital herpes, Valacyclovir is to be given only for 5-7 days. That should be sufficient to heal the sores. Not longer than that is required.
However, since the virus stays in the system forever, so there might be recurrences, which ought to be treated in the same way i.e valacyclovir for 5 days. Only in cases who have very frequent recurrences, is there a need for continuous valacyclovir administration for 6-12 months. Not in all cases.
Unprotected sex caries a risk for transmission of herpes infection to an uninfected sexual partner - even without visible sores- due to asymptomatic shedding of Hsv from genital skin. The risk of transmission is more during a clinical episode of herpes.
Therefore, my advice to you would be to always use a condom and to particularly avoid sex during clinical episodes of herpes- when the risk of transmission is the highest as compared to other times. Even without any visible sores, a condom would reduce the chances of transmission of infection but it would not make it NIL.
