What Causes Pain Under The Left Rib Cage After Gallbladder Removal?

Question: Hi, I have gall bladder removal 2 -months ago. Right now, I have this left sided pain right below my rib cage/stomach area. Pain is around 3/10 but the back part is more worse 7/10. It's sharp kind of pain right below my rib age. I was thinking it might just be a muscle pain but lasted for 5 days now.

I worked up on your case in context to your health concerns of Pain in left stomach, under rib cage.

My Impression-
Based on the facts submitted,You seem to suffer Mostly from-missed out / unattended IBS with C(Constipation)-which causes pain under rib cage in your case.
Besides this pain, pain in left back could be from-bloated abdomen, which could be from accompanied undiagnosed constipated left large Bowel and held up bowel gases(from? undetected accompanied Anal Piles or fissure ),along with flared up IBS(irritable bowel Syndrome)

If this back pain is felt on right back-this could be from PCS(Post-cholecystectomy Syndrome), which is a common cause after gall bladder removed,as is your case.But this possibility does not seem to be there in your case,as you don't have any pains in right back.


Remedy-
Consults with attending/ operating-Surgeon to assess the causes as discussed above.He may rule out other causes after physical examination.
Get Anal and physical checks for its assessment done

Do's to relieve left stomach pain under ribs-
Take Seitz Bath(Warm water with antiseptic) x 2 times a day
Anovate Cream locally x 2-5 days
Dietary do's-
Take Bland liquid diet/like porridges and fruit juices
Avoid Spicy,fried, non-veg diet.
Be on Vegan diet to avoid its recurrence and to avoid further complications out of it.

If no relief-
Get stool tested.
Add Anti-inflammatory Like-Tab Advil-x 2-3 times a day x 5 days
Add Antibiotics-Like Ciplox OZ if need be x 5-7 days
Add Tab Bentyl-antispasmodic -to control pain under ribs and back
Add Probiotic -Cap Sporlac- 1 x 2-3 times x 2 weeks-
Be on Bland diet/yoghurt/butter milk x 2 weeks.
Tab PPI-like pantoprazole and Gastrokinetic-like-domeperidone-to control bloating and stomach pains from accompanied acidity.

Would Need of Planned Surgery-For total Cure of Anal piles/ or fissure, if need be.Other possibilities need to be ruled by Consults with Surgeon.

Actually I had the pain before surgery but told me it's because of my gall stone since radiates to my middle back.it disappeared for a while and is haunting me again. This the pain I was c/o ,which prompted my doctor,s ultrasound and discovery of my gallstones.he even ordered ct scan to rule out kidney stones.


I do have occassional constipation and gas.ill try to reschedule for further eval. Thanks

My Impression-

In the given scenario, My opinion is as follows-
You seem to have 2 pain ailments as follows-
-Based on the facts submitted in updates,the pain felt by you under the left ribs seems to be from the accompanied missed out IBS with Constipation-caused from accompanied? anal piles/fissure.Hence it would need further physical checks from Second Surgeon to get 3rd party transparent opinion on reality,as told herewith.

-But in addition to this,you have confirmed in your updates that-there is another type -as felt in the back side in the middle back, -and is confirmed to be pain similar to pre-operative pain, for which Gall bladder stones were treated by gall bladder removal 2 mths back.
In this scenario,this second pain in middle of back seems to be caused from -PCS(Post-cholecystectomy Syndrome) and is now confirmed to be a possibility.
Hence PCS needs to be investigated and treated in your case.

3rd Possibility-is that both these pains could be caused from PCS ONLY-the one under left ribs-from reflux of bile causing Bile Induced Gastritis,which may give pains in back.The pains in the left lower abdomen seems to be from bile acid induced IBS-causing bloated abdomen with it.

But all this would need physical examination from 3 rd party assessment,for which Second Surgeon needs to be consulted and assessed as suggested above, to rule out various possibilities of your post-operative pains as complained by you.
