Brief Answer:
Rule out the heart , first

Detailed Answer:
Hello and Welcome

I appreciate your concern

It could be arthritis or a musculoskeletal issue that would require NSAIDs, physiotherapy, intra articular cortisone shots , physiotherapy etc. However based upon the description of the symptoms, a cardiac cause should be ruled out first before treating the musculoskeletal / joint pain related to the shoulder. A baseline EKG should be done. If its clear then the shoulder and breast should be examined. Further workup and treatment would depend upon the findings in the physical exam.

Wishing you best of health

Thanks
