What Causes Pain In Penis After Open Inguinal Hernia Surgery?

Question: Dear Doctor

I had open inguinal hernia surgery on Mar 18 2015 (3 months ago).
Today i started feeling pain at the base of my penis near my right testicle. (where the hernia was)

I thought it would have passed. But about four hours later, the pain has intensified and my testicles hurt. The surgery cut also hurts but at the end of the end nearest to the testicles.

My testicles have also become swollen?

Will this pass or is this a sign that something has gone wrong?

Also , previous to this, I was getting pain after ejaculation. At the time of ejaculation, I had no pain. But after about one hour, a pain started at the area of the cut and my testicles. It started as a dull pain, then after about another hour it changed to a throbbing pain at the same base of my penis near the right testicles. When i lay down, the pain lessened. But when I got up from laying down, the pain was very sharp, as if it was dropping to the bottom of the cut. It usually took 24 hours for this pain to subside without any pain killers. I did see the surgeon about this and he said it should pass. This was two weeks ago.

Please advise what i should do

Thanks for your question.
Your pain and swelling that too four months after hernia surgery is quite uncharacteristic. It could either be a muscular strain or some bleed into the area. A difficult problem to deal with would be a Hernia recurrence or an infection.
Both these problems would need to be dealt with.
Is there any redness or fever or difficulty in moving around?
I suggest you consult a Surgeon preferably the Surgeon who operated upon you.
You will require a blood CBC test and probably and Ultrasound of the inguinal and scrotal region.
This will tell us whether there is anything to worry about. With the reports you can let me know.
For infection you will require a course of antibiotics and for hernia recurrence you might require a redo Surgery.
Do let me know the result of the consultation as well as the reports as I can guide you along.

the pain has reduced a litmtle. I don't have difficulty moving around. I don't have any redness. I don't have any fever either. only pain in my testicles and cut.

if it's a redo can I opt to do keyhole surgery or do I have to redo open surgery?

if the pain subsides by the morning , should I still be concerned and see the surgeon or not?

That is good. Redo surgery should be Laparoscopic.

Thanks for the reply.
Good to know that there is no redness or fever.
That would mean that there is no infection.
Recurrence however cannot be ruled out without examination by a Surgeon or an Ultrasound.
Therefore I suggest you consult a Surgeon who can examine you and advice.
As for redo Surgery. If the first surgery was open hernia repair then I would advice a Laparoscopic surgery this time. Which would essentially be by putting scopes with very small incisions. Keyhole surgery that is. Doing another open surgery would not be correct. But all that only after it is found to be recurrent. It is very unlikely that it would have recurred. Specially if they have put a mesh in the earlier surgery.
Do not worry.
You will be fine. But do get examined by a Surgeon so that a recurrence can be ruled out.

thanks doctor. the throbbing pains have gone but I Still have a dull pain around testicles and cut. swelling has gone.

my lower back hurts as well. I am wondering if this could be due to a kidney stone? I do have a history of kidney stones but haven't had one for over 15 years

Ultrasound will be able to visualize any kidney stones.

It is unlikely to be a kidney stone.
There is usually no swelling in kidney stones.
Anyway. when you get yourself checked up you can ask for an Ultrasound of KUB along with the inguinal area. That will clearly tell whether there are kidney stones or not.
It is a minor non invasive investigation and its better to get it done.
The good thing is that the pain has subsided.
But it would be prudent to get yourself examined nonetheless.

Hello. Good you ard getting the Ultrasound done.
CBC will rule out any infection at the hernia repair site. You can also get a Urine RE and ME test done which will be helpful to rule out kidney stones and Urinary infection.

just did ultrasound. there was no sign of hernia or portrusions, no sign of fluids.

The pain in my testes is gone. the pain in my cut is still there but is dull pain. if I press then it hurts. especially near the testes.

I will scan the reports and send to you

That is very good.
Nothing to worry then.
It could be low grade urinary infection or Funiculitis which is causing the pain. No need to worry.
If the pain is severe you could take a course of antibiotics and anti inflammatory medicines.
I usually advice my patients Tab Norfloxacin 400mg twice a day for 10days and also Tab ibuprofen for the pain when it is severe. Drink a lot of fluids and you should be fine.
No surgery needs to be done.

Dear Doctor

I havent taken any of the recommended medications yet. Is Norfloxacin sold over the counter or do i need to see my urologist?

Over the last two days, the pain we almost non existent. However, about two hours, i started to again feel pain at the site the incision. It felt as if it was hardening. I felt like the pain was pricking my right hip.

Then the pain moved lower and lower until it reach my right testes.
Now i have throbbing pain along that route and in my testes.

Since you have pain you should take the pain killers I had advised.

As for Norfloxacin, it is not available over the counter. You need a prescription from a doctor.

But tell me when the Ultrasound was done did the Radiologist comment upon the kidney and Urinary bladder? Did they rule out stones as you were suspecting?
Did you get a CBC test done. I suggest you get it done as also a Urine culture to know whether there is any infection.

It would definitely be useful to see a Urologist as he would be able to examine you.
Specially with the recurrence of pain it would definitely be better to get examined by a Professional.
