What Causes Pain Below Umbilicus?

Question: severe umbilical pain right below the navel and it has gone on for 4 mos, and the drs here do not
know what is causing it. She has lost 40#'s and is a nurse educator herself. I have asked her about
nausea but didn't get a definitive answer. She has had insulin tests and I don't know what else. Her
daughter is a physician

Pain below the umbilicus can be caused by umbilical hernia for example. Especially if she has also kind of lump in the umbilicus. The abdomen can be bloated and she may not be able to pas gas and stool freely.
Please try to palpate her umbilicus. In case of hernia you should be able to feel kind of protrusion or hole. The pain should be more intense when coughing.

She should have at least abdominal ultrasound done to rule it out. If she has also some weight loss, constipation, blood in stool, colonoscopy is a good idea to rule out large intestine issues.
