I have gone through your query and would try to help you in the best possible way. Shrinkage and pain can be due to a number of causes. Most common includes age related, infection (varicocele, hydrocele or cyst). Shrinkage normally occurs with the advancement of age due to physiological change in hormonal levels of body. If i were your treating doctor then i would have advised you an ultrasound of scrotum. It will rule out if there is any infection of testis. In case there is an infection then antibiotic therapy would be prescribed. Meanwhile you can take naproxen thrice a day for pain.
