Brief Answer:
Treat your sciatica.

Detailed Answer:
Hi,
Thank you for using Healthcare Magic. From the information you have provided I think that your symptoms may be related to your spinal problems. Cilostazole is unlikely to help as it is mainly for intermittent claudication which relates to poor blood supply to the legs.
For your sciatica you may take baclofen and gabapentin both at the same time. This combination does make some people drowsy and you should be careful of this if you are driving. However, these two drugs may help any discomfort and reduce muscle spasms and nerve pain associated with your sciatica. I suggest you also take gentle exercise to improve the strength in your core muscles that support your lower back.
As you are suffering symptoms, I suggest you might wish to discuss the possibility of surgery with an orthopedic surgeon.
I hope this is help. If you have any further questions, please do not hesitate to contact me.
