Brief Answer:
needs to be seen as soon as possible

Detailed Answer:
HI, thanks for using healthcare magic

Lower abdominal and testicular pain may be related to one of the following: ( 1) testicular torsion- this is an emergency . It occurs when the spermatic cord is twisted or entangled and cause restricted blood supply to the testes

(2)epididymitis- the epididymis refers to a collection of tightly coiled tubes found on the testes.
In epididymitis- there is infection of these tubes, antibiotics are required

(3)orchitis- this refers to inflammation of the testes that can be due to bacterial or viral infection

(4)hydrocele- a collection of fluid in a sac within the scrotum

(5)testicular tumor

The only way to determine the cause is for him to be seen by his doctor as soon as possible.
Testicular torsion needs to ruled out.

This would be done based on the history, examination findings, urine analysis and imaging may also be suggested.

I hope this helps, feel free to ask any other questions
