Question: My boyfriend has lower abdominal pain and left testicular pain that is agonizing. What could be the cause of it?

''Agonizing'' pain indicates a serious problem.
As the patient has an agonizing pain in the testis, we have to rule out testicular torsion ASAP as if it is there a timely Surgery saves the testis. ( 6 hours in case of Torsion)
Urgent Color Doppler and Ultrasonography examination is needed to see for the torsion and status of the blood supply. If positive for torsion, urgent surgery is required, if not a torsion we all are relieved that there is no emergency.
Other tests of blood, urine to be done later.

The pain in the lower abdomen can be a referred pain from the testis

The other causes for such a pain in the testis and lower abdomen can be due to infection and inflammation of the testis and / or epididymis called Epididymo-orchitis. There is usually associated symptoms of Urinary tract infection like burning urination, chills, fever and pain in abdomen.
Ultrasonography helps here too.

Till you reach ER give scrotal support by lifting up the scrotum, do not give anything orally in anticipation.
