What Causes Low Grade Fever Followed By High Fever At Night In Children?

Question: My 2 year old has had a fever of 102-104 over the past 4 days. For 3 days previous to this she had low grade fever, (101-102). Took her to the dr. bloodwork came back with a slightly low sodium, wbc at 15 and neutrophils at 13. Urine was negative and throat swab was negative for infection. Tonsils are inflamed and white. No antibiotics were given. She is waking up at night with high fever still as well as belly pain and complaining of knee pain. She is not eating much and generally fatigued.
Are we missing something?

Welcome to the HCM
I have gone through your question and completely understand your concerns. Most of the fevers in this age group are due to self limiting viral infections. But since your daughter is having high grade fever for past 4 days along with inflamed tonsils, it's advisable to start her on an antibiotic ( Syp. Cefixime at 15mg/kg/day in 2 divided doses). Antipyretics along with poor oral intake can lead to low sodium, gastritis (belly) pain and knee pain. So don't worry. You may take the following measures to help her out:
1. Start her on Tab. Lansoprazole 30 mg before breakfast once daily. It's for gastritis.
2. Try to offer her regular feeds and fluids including soups, lemonade, buttermilk etc.
3. Blood culture and sensitivity test is recommended before starting the above antibiotics.
4. Acetaminophen or ibuprofen may be given for fever and knee pain.

I would also like to see her blood reports you have mentioned and picture of tonsils. You may upload it for my reference.
