Question: I had a heart attack in 2012,I am on beta blocker like coreg, spirolactone, and crestor. lately I been experiencing low reading shortly after drinking coffee. .chest feall full .I seem to practice deep breathing and letting it slow it helps for a short time .this has been going on now for two month. I am a truck driver. was wondering if it a panic attack or something else.

Regarding your concern, I would explain that it seems more likely to be exacerbation of anxiety: panic attacks (as they are triggered by coffee and relieved by breathing maneuvers).

Nevertheless, as you have a previous history of coronary arteries disorder (cardiac ischemia), it is necessary to closely monitor your blood pressure values for a couple of days and perform a cardiac check up, including a careful physical examination and some medical tests:

- a resting ECG,
- cardiac ultrasound (ECHO),
- fasting blood glucose,
- blood electrolytes level,
- blood lipid profile,
- a screening ecercise cardiac stress test

Only after excluding any cardiac ischemia and any other metabolic disorders, could anxiety disorder remain a rationale responsible factor of your recent complaints.
