Hello Dear. There are various possibilities for your symptoms.

First of all i would have thought of kidney stones or urinary stones. Urinary stones present typically with pain in the back and radiating to testicles. But since you say that urine examination and ultrasound tests are all normal, then that possibility is ruled out.

Another possibility is epididymitis or epididymo-orchitis. That is either infection and inflammation of the epididymis or the epididymis and testes also. In this case urine examination and ultrasound may not show anything.

Treatment for epididymitis involves treating the underlying infection and easing symptoms.

Common treatments include:

1. antibiotics, which are administered for 4 to 6 weeks in chronic epididymitis, and can include doxycycline and ciprofloxacin

2. pain medication, which can be available over-the-counter (ibuprofen) or can require a prescription (codeine or morphine)

3. anti-inflammatory medication like piroxicam (Feldene) or ketorolac (Toradol)
bed rest

Additional treatments may include:

1. elevating the scrotum, for at least two days if possible
2. applying cold packs to the scrotum
3. wearing an athletic cup for support
4. avoiding lifting heavy objects

In cases of an STI, you and your partner should abstain from sexual intercourse until you’ve completed your course of antibiotics and are fully cured.

Hi, in epdidymitis the pain is not constant always, can be on and off, and you need not have temperature. Hot epsom salt bath may relieve your pain or even massaging because it will reduce the inflammation.

Anyways you can take a second opinion by consulting another doctor preferably a surgeon who may examine you, order for urine analysis and re-do ultrasound abdomen if required.

If nothing is found out, probably you can start ignoring the symptoms. Just discuss the possibility of epididymitis with your doctor and take appropriate treatment.
