Question: I"VE HAD PSORIASIS OF SKIN FOR SOME TIME. I HAVE FOUND THAT VICKS VAPOR RUB HAS BEEN THE ONLY TING THAT HAS HELPED. MY DOCTOR HAS PERSCIBED NIASTATIN SHAMPOO BUT IT TURNED MY HAIR ORANGE. I HAVE DONE PRETTY MUCH EVERYTHING SELF REMEDY. I WORK IN THE MEDICAL FIELD AND A TERRIBLE PATIENT. I AM A CAREGIVER NOT GETTER. SPICEY FOODS HAVE A MAJOR REACTION ON ME LOVE THEM BUT KNOW WHAT HAPPENS AFTER. NOW I'VE DISCOVERED NOW IN MY LATER YEARS I HAVE TO BE SO EVER CAREFUL BECAUSE OTHER SYMPTOMS HAVE NOW APPEARED. MY FINGERS SWELL MY FEET ACHE AND SOMETIMES FIND IT VERY DIFFICULT TO MOVE AT ALL TILL I TAKE ADVIL TO START MY DAY. MY HANDS ARE VERY WARM AS ARE MY FEET BURN ALMOST. MY PATIENTS LIKE IT BECAUSE THEY ARE SO WARM. TODAY VERY BAD PAIN ON RIGHT SIDE FLANK PAIN. NOT RELATED TO INJURY BUT VERY PAINFUL. XRAY DONE LARGE LIVER. AFTER CATSCAN DONE LIVER IS NORMAL SMALL KIDNEY STONE 3CM NOT BIG ENOUGH TO CAUSE THE PAIN THAT I HAVE. STOMACH WAS ENLARGED AS WELL BUT NOTHING SIGNIFICANT TO ADDRESS. I AM REALLY FEELING AS THOUGH I NOW HAVE PSORIATIC PSORIASIS. I KNOW MY BODY AND DON'T DO THE DOCTOR THING. JUST REALLY WANT TO WHAT NEEDS DONE TO TREAT IT AND WORK A NORMAL LIFESTYLE. I'M OTHERWISE VERY HEALTHY . IS IT POSSIBLE THAT MY SKIN PSORIASIS HAS NOW GONE TO ARTHRITIC OR OUT OF LINE?

Based on the history you have shared, it seems as if you have already been through alot however the way you are coping with your problems and still insisting in care giving attitude towards other patients while dealing with your illness in indeed commendable.

Based on the recent events, it is quite possible that it may have lead to psoriatic arthropathy and may need proper workup and management in that basis. You may continue using OTC analgesics like acetaminophen for pains and other symptoms but you would eventually need to see a rheumatologist to look carefully and examine you in detail. Other causes which may present with same symptoms like few autoimmune conditions will also be ruled out. Meanwhile keep a close liason with a dermatologist and reluctance to seek medical advice isn't a good way to follow in your case as you have been into this medical field yourself and you would know how important is to pick and treat complications early in the chronic diseases and how important implications it has on over all progression of the disease.

Another possibility of joint pains or skin swellings and burning can all be due to raised uric acid as psoriasis causes rapid turn over of cells which can eventually cause gouty arthropathy or skin nodules. If should be monitored carefully. This increase uric acid can also cause kidney stones as well as primary kidney disease which both can cause lumbar or flank pains.

The flank pains as you mentioned, can be also be due to that calculus. 3 cm is a big size for a renal Stone and can't be ignored at all. Meanwhile Renal function tests should be regularly monitored.

The spicy foods that you mentioned are mostly fat and protien rich so if these are increasing your symptoms so basically they also contribute by increased uric acid production.
