default
Posted on Tue, 14 Jul 2015
Question: I am 84 years old and have had two operations for the removal of bowel cancer over the past 7 years. I am under constant review and appear to be clear currently. However, I developed shingles last week which is a trifle uncomfortable but this evening I seem to have an irregular pulse and feel a little light-headed. Can you give me any clues? I am on medication for high blood pressure, high cholesterol and diabetes 2.

Brief Answer:
Postural hypotension with dehydration possibly.

Detailed Answer:
Hi,
Thanks for asking.
I am Dr. Prakash HM and I will be answering your query.
Based on your query, my opinion is as follows:

1. Symptoms of irregular pulse and light headed feeling is not related to shingles directly.
2. Light headed feeling could be due to low hydration. Take at least 6-8 liters of water per day. Also get your blood pressure evaluated, hopefully no hypotension or hypoglycemia.
3. Low hydration can affect the heart and give a feeling of irregular pulse.
4. If you are monitoring using an ECG machine and are noticing irregular heart beats giving rise to irregular pulse, then electrolyte evaluation is necessary. Serum potassium and sodium requires evaluation.

At present dehydration appear to be a common cause. However, medications other acyclovir, if being taken for pain, will need to be watched.

Move around slowly, so that falls can be prevented. While changing positions, importantly, do move slowly, as postural hypotension can cause light headed feeling and fall.

Improve hydration and nutrition. If it persists, do get the tests done.

Hope it helps.
