What Causes Irregular Patches On Surface Of Tongue?

Question: I have bald patches which keep coming and going on my tongue with a burning feeling , docs said it's Geographic tongue but there is no borders to them and from reading online it resembles Atrophic candidiasis
I also have extreme dry mouth each and every night .
Just had blood tests which showed low Globulin and low platelets!
I had ELISA tests at 7 weeks and 4 months after I may have exposed myself even though a low risk.
What do you think I should do? Or it could be?

I am Dr. Kakkar. I have gone through your concern and I have understood it.

Geographic tongue is irregular patches on the surface of the tongue. This gives it a map-like appearance. Geographic tongue is characterized by areas of atrophy and loss of papillae, leaving a darker red, smooth surface. The depapillated areas are usually well demarcated, and bordered by a slightly raised, white, irregular peripheral zone.

Atrophic candidiasis is usually distinguished from geographic tongue on the basis of the migrating pattern of the lesions and the presence of a whitish border, features which are not present in atrophic glossitis, which instead shows lesions which enlarge rather than migrate.

Regarding your test, a negative ELISA at 4 months after a risky sexual encounter certainly rules out HIV Infection provided you did'nt had any more sexual encounters in between.

-Can you upload a picture of the tongue, so that I can have a look at and help you in a better way?

For geographic tongue, no intervention is required. However for candidal glossitis a course of oral antifungal i.e oral fluconazole 100mg on day 1 followed by 50mg once daily for 14 days, eradicates candida.

Topical clotrimazole mouth paint can be used 3-4 hourly

Hi I have just added a picture of my tongue , can I mention the event of which I am sure started my problems was not sex but a lady rubbing herself around my lower area.
I am also having recurrent mouth ulcers and strange bruising on my body .
I used to be in good health and all of the things started approx 1 to 3 weeks after this event and have never subsided.

Thanks for the picture. The appearance is of geographic tongue. There is a patch of loss of papillae, well defined on the right side of the picture. Rest all the tongue shows normal papillae. No intervention is necessary for geographic tongue.

In atrophic glossitis there is diffuse redness and loss of papillae which gives a smooth appearance to the tongue.

Regarding your sexual encounter, I would like to know whether there was rubbing of genitals against each others? Or was it clothed rubbing with no direct genital to genital contact?

Hi I had shorts on but the lady was putting her hands in my shorts from the front whilst dancing on me ( it was a naked lap dancer) , I didn't look down but worried she may have made contact if my penis was at top of shorts .
She had also previously touched herself if this may pass disease on.
I just have no explanation as to why I have had all these symptoms since

Mouth ulcers/recurrent aphthae can also occur otherwise. I would suggest an oral Vit B complex once daily for a month and the review afterwards.

Regarding your sexual contact, if you are not sure about the kind of contact, you could get yourself tested for STIs at about a months time from this particular sexual encounter. Genital rubbing (skin to skin contact) does pose a significant risk of STIs because there is an exchange of fluids, however it is very unlikely that she could infect you through her hands because HIV, HSV etc cannot survive in dried secretions outside the body.
