What Causes Heavy Menstrual Bleeding?

Question: I am a 24 year old female with no medical problems and no history of irregularity when it comes to my period. I am 5'6" 160lbs and have gained about 20 pounds in the past six months. For the past three months I have not had a full period. Only spotting periodically. Now for the past week I have had an extremely heavy period. I usually only have to change my tampon once every 6-8 hours on a normal period. This time, I am having to change it about every two hours and it does not appear to be slowing down. Should I be worried? On a side note, there is no possibility of pregnancy.

Hi,

Thanks for writing to us.
There are two possibilities in your case-
1) PCOS (polycystic ovarian syndrome)
2) Hormonal imbalance due to use of pills, excessive weight gain, stress and strenuous exercise.

In your case due to excessive weight gain and anovulatory cycles there is an unopposed action of estrogen on endometrial lining. (That is the reason of heavy menses at this time).

In my opinion you should consult a gynecologist and get yourself examined and investigations (blood profile, pelvic ultrasound, thyroid profile and hormonal profile) should be done to rule out the above mentioned causes.

You need to take iron tablets along with tranxemic acid to control bleeding.
You might need anti inflammatory drugs.
In future you would require oral contraceptive pills which would be prescribed by your doctor to regulate your menstrual cycles.
You should do some exercise, yoga and meditation to control weight.
Eat healthy, balanced diet with lots of fruits and vegetables.
