Question: Hello I suffer from Graves' disease Recently suffering g from headaches and left eye pain head feel heavy an have dizziness occasionally. MRI shows small white mTter lesion on left. No tumor no aneurysm etc. opthalmologist noted swollen left optic nerve Graves and thyroid numbers all under control. Feels like sinus full but ENT says sinuses are clear. Woke up with stabbing pain behind left eye No one can give me an answer. I am a health care professional and seek answers

I am Dr.Ajay Panwar,a neurologist here to answer your query.

Graves’s disease can complicate with graves ophthalmopathy in spite of being on anti-thyroid drugs. Graves’s ophthalmopathy results in swelling around optic nerve and swelling of other retro-orbital connective tissue, other soft tissues and extra ocular muscles. Retro-orbital pain can commonly be present in graves ophthalmopathy.
At this stage, it should respond well to 4-8 weeks of corticosteroids therapy. In advanced stage it may need radiotherapy. Please visit an ophthalmologist for opinion. Graves’s ophthalmopathy does not respond to anti-thyroid drugs.

White matter changes in brain MRI are common in Graves’s disease and reversible with corticosteroids therapy.
