Question: My mother has Lupus or SLE as well as many other female relatives in the family. I am 25yrs old, female, and my first bladder infection was when I was 6 yrs. old which caused me to bed wet suddenly, and that's how it was picked up. Over the last 8 yrs. I have had constant uti infections and sweat profusely at night, I wake up drenched. From time to time I suffer pelvic, and groin pain, as well as lower back pain. I get dizzy spells, and have fainted a few times, and have blacked out on occasion and suffer extreme tiredness and can sleep for days. My shins ache and hurt to touch and my knee slips out of joint regularly. I get tingling in my feet and fingers. Over the last year I have developed foul smelling, brownish, oily looking urine. No amount of antibiotics help and have had many urine tests. I often feel nauseated, either have constipation or diarreoah, and just feel listless and depressed.

Foul smelling, brownish, oily looking urine is never normal. You surely need an ultrasound of abdomen/pelvis and urinalysis again. This would give some hint but a Physical examination is mandatory to assess this since there are variety of other causes that needs to be ruled out. Apart from uti, things like glomerulonephritis should be excluded as you have a strong family history of Lupus.

Many of the other symptoms you mentioned like sweat profusely at night, dizzy spells, extreme tiredness etc suggest that you may be having chronic infection which should be assessed stating with a complete blood picture which again gives a hint of kind of infection it may be.

I think that a coordinated consultation with a Nephrologist and gastroenterologist would be very helpful in your case
