What Causes Erectile Dysfunction While On Losartan And Allopurinol For Gout?

Question: I am taking losartan hctz 100/12.5 and allopurinol 300mg for gout. I am having trouble getting a full erection and it is causing problems in the sex arena. What can I do?

I appreciate your concern

Allopurinol is associated with the adverse effect of erectile dysfunction. It is not clear whether gout has a direct role leading to erectile dysfunction.

Losratan, in fact improves sexual dysfunction in men who are already on anti hypertensive medication such as calcium channel blockers or beta blockers.

If the problem persists then sildenafil or tadalafil can be prescribed, in consultation with your doctor. A fasting lipid profile and blood sugar level should be obtained to screen for high LDL, cholesterol, TG and diabetes.

TGs are triglycerides.

If the TGs, LDL and cholesterol are raised then statins can be advised which prevent atherosclerosis progression and have a positive effect on erectile dysfunction.

Diabetes can also lead to ED and needs to be screened for. Diabetes classically presents with polyuria, polydipsia and polyphagia. However, a person may be asymptomatic with no symptoms at all.

If there is no organic/physical cause for the erectile dysfunction and there is associated stress or performance anxiety then BCT (Behavioral cognitive therapy), psychotherapy and consultation with a psychiatrist or sexiologist can help. There may ne no need of medication if these measures are used primarily.
