Posted on Wed, 20 Apr 2016
Question: Hi, i am 46 year old female disgnosed with elevated bp with high pulse. My pulse reaches 134 sleeping and 124 resting. Blood pressure will be only slightly elevated. I sweat profusely just doing mild housework. I am on generic lopressor. Have been fighting chronic headaches, but those have lessened. I have had a partial hystorectomy due to fibroid cysts, back in 2008. I do not struggle with anxiety or excessive worry. No depression. My pulse and extreme sweating are my main concern. My thyroid has always tested normal. My morning temp os 97.5 almost always. Thanks!

Detailed Answer:
Hello!

Thank you for asking on HCM!

Regarding your concern, I would explain that it is important performing a comprehensive differential diagnosis of different causes that may lead to your symptomatology:

- an inflammatory disorder, including autoimmune disorders
- pheochromocytoma or adrenal gland dysfunction (although pheochromocytoma would be associated to uncontrolled high blood pressure)
- a chronic infection (Lyme disease, TBC, etc.)
- early menopause

I would recommend consulting with your GP for a careful physical exam, and some tests to investigate for the possible cause:

- a chest X ray study
- an abdominal ultrasound
- an ambulatory 24-48 hours ECG monitoring to examine your heart rhythm trends during the day
- lab blood tests (complete blood count, PCR, sedimentation rate, blood electrolytes, kidney and liver function tests, fasting glucose, sexual hormones plasma levels, blood lipid profile).
- urinary metanephrines

Further tests for possible infectious disease (gama INF, Mantoux test, Lyme disease tests) would be needed.

You should discuss with your doctor on the above issues.

Hope to have been helpful!

Kind regards,

Dr. Iliri
