Question: I went to my doctor today with some discomfort while urinating. More of a tingling sensation. He did a urinalysis and there were no nitrates, 10 protein, wbc 25, trace rbc. He was confident that this was more of an irritation than infection because the wbc wasn't real high and no nitrates. He didn't give antibiotics but said to drink alot of water. I am also using cranberry.

Dear patient,

i agree with your doc about the interpretation of your urine analysis. To make sure i would do an additional bacterial culture though (there is bacterial infections with negative nitrate). If bacterial culture is really negative and if this should persist for more than a week if you were my patient i'd recommend an ultrasound of the urine bladder and kidney to rule out any other condition than infection.

So you agree with my doctor but he didn't think I needed more tests unless I got worse.

And he didn't mention a culture because no nitrates

sorry for the delay, it was 3 at night in Germany where Iam wrting you from. So as I said there are infections with no nitrates. And I would recommend a culture. So you are correct in that point I disagree with your doc.

Do I need to call him and ask for this now or do what he recommended for a few days and call and ask for the culture if nothing improves or if it worsens?

I also have had back pain. It was on the right side and felt like sciatica, affected by movement etc. Now it is on the left (right side pain is gone) and I feel it on the side and low back and butt. It also is affected by movement and I can get in positions where it doesn't hurt. When I relax it's better. I'm extremely tense and afraid of all this. My doctor thumped on my back on both sides and said it had to be muscular and not related to other symptoms. He also noted the muscles were tense. But I can't seem to let it go and the more I focus, the worse it gets. I've been on the internet and now I'm worried about cancer too. My doctor was confident there was not kidney involvement. Does that sound right?

to me that sounds more like muscular back pain. Although this can only be said for sure through clinical examination and (if persists longer) x ray/ mri (and ruling out kidney condition). Usually this gets better with physical therapy/ back exercises, local warmth and pain medication.

I went to another doctor for another opinion. Today my wbc was 75 and protein 100! Really freaked me out. But it didn't freak out the doctor. Even though still no nitrates, she said symptoms and urinalysis indicate infection and she is treating with cephphlexin. She seems confident that the protein is consistent with the wbc and the infection and it will come down when it's treated. I'm still worried about the protein. However, if it was true kidney damage, wouldn't it be gradual and not a sudden increase? Also, she examined my back and also the eleven it's muscular. She said I have nothing to be worried about, just go about regular routine, exercise, everything. She is also culturing. Does this sound right? Really obsessing about this. I was surprised the urine was worse. It looked better and my symptoms are no worse.

Dear patient,

no need to worry, protein in this range can be normal with infection. It does not always correlate with symptom intensity. You are on proper antibiotics now and this should get better soon. Good thing a culture was taken to evaluate treatment afterwards.
Do a urine test again when infection is over and it will be very likely be normal. Back pain might be different issue as also examined by 2 different doctors now, to be treated as advised above.
