Question: Hi Doctor, On March 23rd 2014 I have posted my question and based on my symptoms I got a reply from you that my case is related to GERD and you suggested for endoscopy. Next week I am undergoing endoscopy. I have one question doctor. Last week I went to another doctor and she was saying it may be a case of peptic ulcer and she also suggested gastroscopy. My question is for last 3 days I am finding difficulty in passing stools. I have to push for some time. Its hurting my anal area. It never happened to me. I think its called constipation. I don't know why its happening suddenly. Could you please let me know. Do I need to visit my GP immediately. May I know the treatment for this and any medicines to be taken. Thanks for your time. Regards, XXXXXXX

Brief Answer: yes its constipation only Detailed Answer: Hi XXXXXXX Glad to hear from you again. Yes you are having constipation. And that is due to less fiber in your diet. Include some fiber rich food in your diet like green vegetables, fruits, bananas, oats, cereals, etc. For now you can take a laxative to help pass out the stools. If that doesn't helps then you need to be given enema. But try taking a laxative first and see. If pain is more, you can apply an anaesthetic ointment at the anal opening to ease off the pain. Its not an emergency at present but if you need enema and you are unable to administer it yourself at home and you can either call a nurse at your home for it or got to an ER where they will administer it to you. Its not a difficult task. It comes in a poly pouch with a ling nozzel. Just cul the tip of the nozzel and insert it in the anal opening as deep as you can and squeeze the pouch. Thats it. Hi Doctor, Thank you very much doctor. Sorry for my long mail. From last 1 month in order to avoid more burping I have been taking only curd and eggs (like omlette) and very few times (4 to 6 times) I have taken food in restaurents (veg or non veg curries). Is that the reason for low fiber in my food. Doctor could you please let me know if this is the reason for constipation. Doctor may i know what is meant by laxative??. Does this means the rich fiber food you have mentioned such as "diet like green vegetables, fruits, bananas, oats, cereals, etc.". Please let me know. I have few questions doctor: As I already mentioned in my last mail, last week when I visited an emergency doctor she suspected my condition as peptic ulcer. Please find the attached doctor report. I searched in internet and I found that H pylori bacteria causes it. One of my doctors previously visited also told strongly that H pylori is causing burping but I ignore about this at that time Generally in the weekends I used to get more burping than in week days. I got a doubt on the drinking water in my home and I completely avoided taking drinking water in my home and I have taken water bottles from stores and taken water from my office. From the day I avoided drinking water in my home I felt really well and I felt 50 to 60 % better (in fact lot better) and I dont have that much burping now which was experienced before. Generally in the weekends I used to get more burping and very uncomfortable but I was feeling lot lot better when I have taken water from outside compared to when I used to drink water from home. I stay in australia and we usually directly take tap water in kitchen as drinking water. Also doctor when I am taking food outside I was not that much uncomfortable even though I have taken veg curries but when I was taking rice cooked with home water I started to get more burping. And especially in weekends as I spend whole time in home the condition becomes worse. I suspect the tap water I am drinking in my new home is contaminated and I feel that H pylori bacteria entered into my body because of that. Doctor Could you please let me know if tap water is the cause for my condition because burping started after I moved into this new home only. Before that I have stayed in other houses around 3 months but nothing happened. Sorry for my long mail. Thanks for your time. Thanks and Regards, XXXXXXX

Brief Answer: report is normal Detailed Answer: Hi XXXXXXX Your report is all normal. Curds, eggs, non-veg are all very low in fibers. Green leafy vegetables are rich in that. So your diet is low in fibers. Laxatives are a class of medicines that makes your stools soft and help in its evacuation. I don't know if its available at your place without a prescription or not. So first check at your local pharmacy and if prescription is needed then you need to contact your local doctor. After your constipation gets cured, then for prevention of further episodes add more fibers in your diet as i had suggested in my previous reply. Now regarding your burping and your water. Maybe your home water is a bit hard and rich in carbonates which in your stomach due to acid action release carbon dioxide leading to burping. So either you can either continue with your bottled water or get a water filter installed which can reduce the water hardness. Cuming to H. Pylori, its presence can easily be detected during your endoscopy. And if present, it can be cured with medicines. So get the endoscopy done, it will reveal everything what is going on inside. H.pylori usually does not comes from contaminated water. And its infection is not detected by blood tests. Its detected either by endoscopy or a special breath test done after ingesting radio labelled urea. Hope i have answered to all your queries. If you have further queries, i will be glad to answer. Brief Answer: its normal Detailed Answer: Hi XXXXXXX Your gastroscopy is normal. Means there is no gastritis, gerd or hiatus hernia. Biopsy report will tell about h.pylori infection. So wait for that. Rest there is nothing to be worried about. Its just your water only that needs to be corrected. Regards Dr. Ashish Verma
