What Causes Delayed Menstruation Post Intake Of I-pill?

Question: I am a 30 years old lady.
I suffered from irregular periods till 2011 and after that i started getting regular periods.
However, my cycle was nearly 40 days.

But the irregulary started again from may this year.
Last time I had periods was on 12 th May but I didn't get periods yet.

Also, I happened to have unprotected sex on July 13th.
I did take emergency pill the next day 14th July (within 24 hours) but I have no sign of periods even after 11 days.
I do suffer some stomach aches at times and other physical problems but no trace of bleeding.
Am I preganant or it is normal not to bleed even after 11 days of having an ipill.

Detailed Answer:
Dear Piu,

You are concerned about this delay in menses for mainly two reasons:

1. You have a history of irregular menses in the past and so are worried of its recurrence.

2. You had unprotected intercourse in this cycle and are worried about pregnancy.

Let me inform you Piu, one delay in menses is not considered as irregularity in menses.

You had taken emergency contraceptive pill within 24 hours of unprotected sex which offers more than 90% protection against pregnancy. So the probability of pregnancy is very low. Still, if you are so much concerned about it, you may perform pregnancy test on your overnight first morning urine sample. This will clarify your doubts about pregnancy.

Emergency contraceptive pills may cause some amount of bleeding within few days of consumption. No cause of concern. Moreover, the subsequent menses after the pill can get delayed due to the hormone it contains. However, this delay is usually not more than a week to 10 days.

If your pregnancy test is negative (you may upload the picture for my review), and still if you do not get the menses, you may take Tab. Deviry for 5 days. Then within 4-7 days you would get your menses.

Meanwhile, please inform me the details of your menstrual pattern - age of menarche, days of bleeding, periodicity, frequency, amount of bleeding, pain factor, etc. along with your marital and obstetric history which will allow us to discuss more scientifically further.

I will take the home pregnancy test.
However, I was wondering when should I take it as I had unprotected sex 12 days back.

Also, the details of my menstrual pattern are mentioned below.
age of menarche : 11 years and 2 months
days of bleeding : 4 to 5 days
frequency: 40 days
amount of bleeding : Heavy on 2nd day and moderate on other days
pain : first day is really painful. there is no pain on other days usually

I have never been pregnant before.
I am engaged and marriage is on the cards next year beginning.

At the outset, let me congratulate you for your forthcoming marriage next year.

Home pregnancy test gives best and reliable results one week after the missed period or about 21 days after the conception. Done earlier, there is always a possibility of false negative results. If you want to have more reliable results still earlier, you may get Beta hCG test done on your blood 10 days after the last intercourse you had. This is very sensitive and specific test. In non-pregnant state, hCG levels are under 5 mIU/ml while levels more than 25 mIU/ml are diagnostic or pregnancy.

Well you had menarche at an expected age. The pain you are getting during first 14 hours is indicating that you are ovulating. Now the question of heavy bleeding on second day. I feel this is due to your prolonged 40 days cycle.

However, if your cycle is having regularity of 40 days, I would say, it is regularly irregular. Would you let me know your height and weight?

With 40 days cycle, your day of ovulation which would be 14 days prior to next cycle will be day 26. While planning for child bearing, this information may be of use to you.

If you want to have shorter frequency of your cycles to around 30 days, you may take birth control pills regularly for 3 months. From your description, it seems that you are Oestrogen dominant girl. (If you inform me your phenotype, I can
guess this more accurately.) So Progesterone dominant pills will suit you better and will minimize your bleeding also.
