What Causes Coughing Up Of Blood?

Question: Hello,

I have a VERY complicated medical history and Doctors can not work me out at all - i still cough up blood and I have very high uric acid (even on 300mg allopurinol) I have terrible ABG results and low O2 saturation and some kind of systemic inflammatory response - I do have Psoriatic Arthritus - but I just cant seem to get my Uric Acid levels down, I avoid all Purine rich foods and do not drink alcohol or eat a lot of red meat. My health issues are quite serious indeed and no Doctor can work out what is going on with me - I have Squamous Metaplasia in my airways and always have pain in my chest - D/Dimer is always positive - CRP is sometimes high but Liver and Heart all seem fine, excercise stress tests are ok, along with Heart Ultrasounds and bubble studdies, etc, I do have a positve shunt study showing O2 problems - but something just keeps raising the Uric Acid level - no matter what dose I am on of he allopurinol - I need some help with this

Hi,

I understand your situation and empathize it. You seem to be lost hope because of the complex medical history. The doctors are not able to come to a conclusion and offer a treatment plan, that shows the intricacy of the problem.

Since the main organs like heart, liver, kidney are fine, I do not think there is life threatening event soon. You are not sure about the lung condition and you seem to be coughing up blood very often. The oxygen sats are falling. So there seems to be Inflammatory lung disease.

The various medications that you take might be interfering with the Allopurinol and not letting the Uric acid levels to come down. You should enquire your doctor. You may possible increase the Allopurinol to 800mg or change to xanthine oxidase inhibitors.

Was a lung biopsy and/or Bronchoscopy done to check for any leaking capillary?

At this time without panic I suggest you to go ahead with symptomatic therapy in the absence of conclusive diagnosis and treatment plan. However it also helps in monitoring the deterioration.

Hello Doctor,

Yes I have had many chest xrays, Bronch's Endoscopies, CT's and Mri's - all they ever find is a strange build up in the bottom of my lungs and chronic inflammation, my lung biopsies and VAT did not show any major issues at all but did find blood and squamous metaplasia.

I am not the kind of person to worry, so please do not think I am panicking - I do however wonder if the Uric Acid level is from the low O2 and cell death, my ABG's are not good and the positive O2 shunt study shows reason for thinking of venous issues.

There was no mention of leaking cappliery from last Bronch, can you please outline what else can cause high uric acid?

Diet and medicines are the ones that I could think of now as the cause for high uric acid.

At this point I am unable to focus on any particular problem but Inflammatory vasculitis. I generally do not follow such complex cases given my expertise and specialty practice.
