What Causes Coughing And Vomiting After Having Breakfast?

Question: My son has been suffering from itching like cough and vomits whatever he intakes after breakfast and snacks only and not after lunch or dinner; he is 7 years old; Doctor has given him monteir, cetrizine and some digestive enzymes for last two months; pl guide me

By what you quote I feel what your kid could be having viral associated wheeze or multi triggered wheeze. I have a few questions for you -

Questions:
1. How many days per month does he cough or feel breathless?
2. How many nights per month does his sleep get disturbed due to above symptoms?
3. Does he feel breathless when she runs around or plays with other kids?
4. Are the symptoms when there are seasonal changes?
5. Is there any family history of asthma or any other sort of allergies like skin allergy etc.?
6. Is the cough always associated with fever?

If your answer is yes for any of the above questions, your kid might be having viral associated wheezing or multi triggered wheezing and I suggest you meet a paediatric pulmonologist who is near your place.

Till date we never found breathless condition in my kid; neither when he has cough or during any kind of running activities. He always has a sound sleep. I have mild asthma and sometimes dust allergy. He doesn't have fever for your last question. But he feels some sort of irritation due to which he coughs all the time like something has got chocked inside his neck (as we all do when we clear our voice after cough for which we normally take vicks or any other cough logenges, hope I made it clear). But my concern is his vomiting which he always does after having any breakfast and snacks only and not after lunch or dinner. This problem has also been for last two months for which my pediatrics has given some digestive enzymes like optifate. Kindly please help.

Could be GER - therapeutic trial is worthwhile

Detailed Answer:
Hi...Thanks for your answers. You have made it clear. With the history your gave about your kid and the detailed answers to my questions, I now think that all this could be due to gastroesophageal reflux (GER) which can aggravate or cause this type of throat irritation and cough. This GER can get aggravated with the snacks due to artificial additives and oil or some chemicals (to make them tastier) in them.

In GER, the acid in the stomach travels up the food pipe (esophagus) and irritates the back of the throat and it will in turn cause severe cough followed by vomiting.

The best way to find out is to give a therapeutic trial of anti reflux medications with drugs like Omeprazole and Domstal for 8 weeks and see for the response. If the cough gets better in a week or two then the diagnosis is almost confirmed. No need for any further tests.

But one significant positive aspect for this being an allergic phenomenon is that there is a family history of dust allergy. This too can cause cough and that in turn will cause cough induced vomiting.

With this - I suggest you get back to your paediatrician with these tips and get the treatment for GER as suggested.

Kindly pl. let me know if there are side effects of using Monteir-5 tab. and what are they as doctors have given him for last one month; how long these tab can be taken ? and the medicines which is prescribed by my pediatrics along with monteir-5 are cetrizine, Lenzol tab (may be for vomiting) and an digestive enzyme after night are fine ? should I continue with that or go for a change ? I will be highly oblidged if I could get ur mail id or cell no. also.....Looking forward for your reponse

Suggestions on Montair and treatment

Hi...Thanks for you trust in me.

Coming to your queries -
1. Montair is Monteleukast - a drug which will help controlling allergic symptoms on the long turn. This won't have immediate effect, but on using long term like 6-8 weeks the allergic symptoms will get reduced. Usually it doesn't have any side effects - the rare ones are anxiety and rash.

2. These can be taken for 8 weeks and if not much improvement can be discontinued.

3. Lanzol is the same as I suggested (like Omeprazole), but digestive enzymes have no role in treatment (no scientific basis).

4. I suggest you discontinue digestive enzymes and go ahead and start Domstal after consulting your treating doctor.
