Question: Good afternoon Dr. I am 31 years old perfectly fit person. I am 5'8 and weigh 148lbs. But today in a very long time I am feeling subtle and consecutive thumps around my heart area. I get one every so often but it goes away. I don't know if it's a minor panic attack I don't have any more symptoms but those small very subtle thumps every 5 or so minutes. What should I do or can I do? They usually go away by themselves, but I get scared within the process :-( Thank you so much -XXXX

Detailed Answer:
Hello!

Welcome and thank you for asking on HCM!

Regarding your concern I would explain that your symptoms seem to be related to anxiety.

Nevertheless, I would recommend consulting with your attending physician and performing further tests to exclude other possible causes of this symptomatology:
- a chest x-ray study
- an ambulatory 24-48hours ECG monitoring to examine your heart rhythm trends for a prolonged period and exclude possible cardiac arrhythmia
- complete blood count for anemia
- blood electrolytes for possible electrolytes imbalance
- thyroid hormone levels for possible thyroid dysfunction .

If all the above tests result normal, you should consider anxiety as the main cause of your troubles. In such case, you should consult with a specialist of this field to help you manage your anxiety.

Hope to have been helpful!

Kind regards,

Dr. Iliri
