Brief Answer:
Most probably anal fissure

Detailed Answer:
Hi.
Thanks for your query.
To recapitulate: Female/68 - Hemorrhoids - whole life - no rectal bleeding - only pain - has constipation and noted blood on toilet paper - not in the stool - Pharmacist suggested Colace - wants to know whether this is the way - 68 and swinging ...

My thoughts as per the history your have provided:
Pain- is indicative of a fissure in ano. There may be a single or two small cuts at the anal verge and on passing hard stool they re-open easily to cause pain and occasionally streaks of blood on stool or few drops of blood after passing stool in the pan or on the tissue paper.

Surely, the mainstay of treatment is the laxative, colace.
Additional things to do are:
Sitz bath in a tub with warm water.
Make a list of the foods, beverages, food additive and anything that causes constipation/hard stool and avoid them altogether.
Secondly make a list of the foods and beverages that suit you the most, help you to have smooth evacuation and to consume such things only.
Get a reference for a General Surgeon for clinical evaluation, per-rectal examination if the pain is tolerable, ano-proctoscopy and colonoscopy.
Further treatment as per the findings.
If the medical treatment fails to get a complete relief a small procedure may be needed.

I hope this answer helps you, please feel free to ask for further relevant queries if you feel that thee is a gap of communication.
