Brief Answer:
Most probably a fissure in ano.

Detailed Answer:
Hi.
Thanks for your query, albeit short and specific.

The commonest reasons for blood in stool with sharp stabbing pain are:
Fissure in ano. This develops as a sharp small cut in the most sensitive part of the outer part of anal canal and hence gives blood as well as severe pain which may last for half an hour to a few hours. This usually follows a hard stool.
I would advise him the following:
High fiber diet.
Avoid all the foods and beverages and stress-like conditions which gives constipation.
Laxative for softening the stool- never allow the dose too much to cause liquid stools.
Sitz bath- very important- just before passing a stool and after passing a stool.
Clean the area very clear of the fecal soiling that remains after cleaning; use cotton soaked in the same water to clean.

Another rare reason can be prolapsed piles/hemorrhoids. Seen and felt as a mass.
Procto-colitis means inflammation of the rectum and colon can give similar picture in a few cases.

He can obviously get examined and get a prescription based medicines after seeing a Doctor.

I hope this answer helps, please feel free to ask for further relevant queries if you feel that there is a gap of communication.
