What Causes " Endometrium Borderline Thickened For A Post Menopausal Woman" Mean?

Question: I had a transvaginal/abdominal ultrasound done 4 weeks ago today. I was told it was unremarkable and that the endometrial lining was 5mm. I asked for the results which I received today. Could you please explain this to me.
Exam:Pelvic ultrasound, transabdominal and transvaginal
Clinical History: Uterine bleeding
Compariosn: No priors
Findings: Uterus measures 3x4x9.2 cm. Transvaginally the endometrium measures approx. 5mm. Left ovary 1.5x0.8x1.3 cm. Right ovary not distinctly visualized. No free fluid.
Impression: Endometrium borderline thickened for a post menopausal woman without separable mass lesion. Right ovary not distintly visualized.
Could you break this down for me please? Thank you so much!

At age 60, when you are definitely post menopausal, the thickness of the endometrial lining, usually becomes less than 4 cm. This occurs due to absence of certain hormones as menses have stopped. However, after menopause, if bleeding occurs, its imperative that the cause be determined. Most commonly cancer of the uterine lining needs to be ruled out. In such a case, the first investigation done is the ultrasound. Here, we can determine the ET and uterine size etc. In your report, the endometrial thickness is 5cm, slightly over the normal range. This is called endometrial hyperplasia. Commonly occurs due to malignancy and less commonly due to erratic hormonal imbalance. Its very important that the cause be found out. You doctor may suggest a D/C biopsy first to find out of there is any sign of malignancy.

My doctor told me if I have anymore bleeding or pain, to let him know. He has been my dr for 35 years. Another doctor on my thought all was OK and recommended a biopsy to ease my mind. No one said anything about abdominal hyperplasia. The bleeding was just pink on the toilet paper after urination back in Oct during a stressful time, I did not have that pink episode again until 2 weeks prior to the ultrasounds. It was 1 time. Also, I have gained 50 lbs in the past 3 years. That to I was told can cause endometrial lining to thicken. I am losing it. Also, I don't understand what they meant concerning one of the ovaries. Why wouldn't they be able to see it?? Thank you

Firstly, endometrial hyperplsia is the medical term for thickened endometrial lining. If you say that you have had just one episode of bleeding then you can wait. However, if this ultrasound report is recent, that means that the endometrium is thickened again, and its possible you may have bleeding episode again. Contrary to what you have been told, endometrium thickening depend on hormones and not body weight. If the lining is thickened then its due to hormonal effect. In my opinion, a biopsy would be advisable.
With regard to the ovary, sometimes after menopause, ovary may be shrunken so its difficult to see. Or it could be an observer error due to position etc. Its not a major problem.
