Posted on Tue, 13 Jan 2015
Question: For Dr Rovena Murati.

i could not finish my questions in previous conversation. i have some questions 1st is i am using probiotics as per your suggestions and after that from last one and half month i am feeling very much improvement in stomach. i an appetite , pain and soft stool (which i have not from last 3 weeks). Now my question is i have a fear that when i stop these probiotics i may again start feeling the symptoms . or probiotics does not behave like this.

and my 2nd question is related to your lat answer as you told about muscle pain and pain in the inner feet muscle . this feel muscle pain i felt when first time i observe stomach upset and soft stool . but as i told you at that time i was doing running exercise in gym. i think its due to that . and that pain reduced with time and near about finished in one month. so i think its not due to neurological problem. am i right?

Hello again XXXX

I apologize for the delay in answering you.

I read carefully your queries.
You don't need to worry about probiotics. They don't have any side effects so you can use them long term.I would suggest to continue using probiotics for some more months (2-3 months).
After stopping probiotics you can continue taking foods that act like probiotics like yogurt ,diary and eat foods rich in fibers to have normal stools.
If after stopping probiotics you start to experience again IBS symptoms you can start again Probiotics at any time.

Probiotics act by normalizing the bacterial flora of gastrointestinal tract .
Normaly the microbial flora of GI tract contains "good" microorganisms and "bad" ones that are in a certain balance.
There are many factors that can disrupt this balance decreasing the number of good bacteria in the gut and increasing the number of bad ones.
This causes gastrointestinal disturbances like bloating, gurgling, constipation, diarrhea, abdominal cramps etc.
If the factors that disrupt the bacterial balance in the gut persist you may need to take probiotics for long time or time to time when you have symptoms.

Regarding the second question ,such kind of pain like yours is not due to neurological problem. If it was it would have a specific localization and a specific distribution that would follow the anatomy of nerve distribution.
You muscle pain most likely was due to exercises.
Usually neurological pain is a very strong pain and it is easily distinguished from muscle pain that is usually a mild pain ,that increases with touch and movement .

If you have any other question ,please feel free to ask for more information.

Wish you all the best,
Regards

Thanks for answer .
as you told that these twitching are mostly not dangerous. but i observe not in a specific area. its all over body. is it same thing?
all over body means i may observe in uper or lower back legs arms etc.
2nd i already told you many times about a pain under lower ribs in right or left both sides. which i observe near about one month before( last time). does it was related to neurological or any lump node condition.?

Detailed Answer:
Hello XXXX

Muscle twitching all over the body are less likely to be due to neurological condition. Most likely they are just benign fasciculations that are triggered by anxiety.
Other possible causes are thyroid problems, electrolyte imbalance, vitamin deficiency etc.

The pain under your ribs ,in both sides, more likely is just a muscle pain or with abdominal cramps due to IBS or acid reflux. There is no neurological disease that can cause such symptoms.
Such pain it is not related to any disease that also causes swollen lymph nodes.

Now my main problem are these twitchings bcz i already told you that my stomach is very much better. But these on anf off twitching gives tention. Sometimes i did not felt XXXXXXX hours. I felt that when i started multivitamins these reduced but that also effecting on stomach.i dont know what i should do. If i have thyroid problem is there any tip to reduce that effects? And what could be the reason behind the starting of this problem? And does this weight loss also related to thyroid? And slight under chin pains are related with this problem? That i sometimes feel.

One thing i have to mention here. My muscle twitching start when first time i have stomach upset. Is there any relation?

Let me know if you need further help.
Take care.

Hello again XXXX

The fact that these muscle twitching start with your stomach upset may indicate that most likely your symptoms are triggered from anxiety,or are due to vitamin deficiency ,thyroid problems .
The first thing you should do is to consult a physician and get the tests done to determine the cause of your symptoms.
If these are caused from thyroid condition ,it can be treated with medications. There is no tip that would reduce your symptoms apart from specific treatment (hormone replacement in hypothyroidism and medication to suppress hormonal production in hyperthyroidism).
Usually hyperthyroidism can cause weight loss and such twitching sensations on your body.
It doesn't cause pain under chin but can cause some kind of discomfort in the neck .
At this point the only way to determine the cause of your symptoms is to get examined by a physician and get the tests done.

Hello again XXXX

Hyperthyroidism is due to overproduction of thyroid hormones but there are different thyroid condition with different causes that can increase hormone production(hyperthyroidism) or decrease in hormone production (hypothyroidism).
Some diseases are auto-immune conditions. In these diseases the body doesn't recognize some structural parts of thyroid gland as part of it and produces antibodies to fight them.
Other possible causes are :inflammation of thyroid gland ,viral or bacterial infection of thyroid gland,Jodine deficiency (goitre), Toxic adenomas (nodules that develop in thyroid gland ).

Hyperthyroidism can cause increased appetite with continuous weight loss and hypothyroidism cause loss of appetite with unexplained weight gain.
Usually thyroid conditions causes progressive symptoms that get worse until the treatment is started.
Since your symptoms seems to have improved over time, you might not have thyroid problems.
You can go for the tests just to exclude possible condition.

Take care.

Detailed Answer:
Hello XXXX

I am sorry that such findings causes you anxiety and tension. They are really benign findings that are not associated with any kind or disease.

Cheery angiomas are very common in people. They are harmless and you can consider them just like moles. They are not related to any blood disorders.
The red dot in your palm is not a symptoms of blood problems.
I can tell you for sure that every possible blood disorder would have caused you very different and obvious symptoms and cherry angiomas and the red dot in palm have nothing to do with blood disorders.

It is normal to have wrinkle skin mostly in inner part of thighs when you have a rapid weight loss. The same thing happens with stretch marks when you gain excessive weight for a short period of time. This is physiological.
This will reduce with weight gain and regular exercise .You don't need to worry.

Let me know if I can help you further.
Take care,

Dr.Rovena

dr just a last question i have eyelid twitching is also same thing? means liker other muscle twitching? now these are the two most fearing things that i have. one muscle twitching and vibration and other is weight loss . i have a fear to measure the weight . i am eating well from last one month . how much time it will take to gain normal weight again? i was thinking i have constant weight but these wrinkles in inner thighs give tension now of reducing weight.
my one question is about probiotics. does they increase appetite because i thought that when i leave them i will reduce my appetite again. is it ture?

Brief Answer:
doesn't seem to be of concern.

Detailed Answer:
Hello !

Eyelid twitching also known as eyelid myokymia are pretty common and usually benign and not associated with any disease.
They are voluntary contractions or eyelid muscles. Most commonly are caused from stress, alcohol intake, lack of sleep, dry eyes.
Another very common cause is eye strain due to computer overuse(it also causes dry eyes that can cause eyelid twitching).
Usually they may last form some days to few weeks. If it persist more than few weeks you may need an eye exam because you may need vision correction.
It might be the same like the twitching in other parts of your body and they are triggered from stress, fatigue, lack of sleep etc.

Regarding you weight you don't need to worry. It is not meant that you should gain weight.Important is to have a constant weight according to the calories that you take a day.
You just need to give it some more time and if you continue eating well you will achieve the weight you want. The wrinkles in your inner thighs are due to initial weight loss. They will go away gradually with time as you gain weight.
Don't need to be anxious about this.

Probiotics have an indirect effect on appetite since they improve your gastrointestinal symptoms and this increases your appetite. You can continue with Probiotics for some moths and when you feel totally OK and have gained weight you may stop them.
Stopping probiotics won't reduce your appetite .If you start experiencing IBS symptoms again ,this may reduce your appetite. In this case you can start taking probiotics again .

Hope this helps.
Please feel free to ask if you have any other question.

Take care,
Dr.Rovena
