Question: Feel tightness outside throat area & into chest.
Worse toward evening.

Hi, I have gone through your question and understand your concerns.

One possible explanation of your symptoms may be gastroesophageal reflux or turning back of stomach acid into esophagus, especially if this happens when you lay down.

This condition is treated with antiacid drugs (Pantoprasole eg.), diet modifications and sleeping with two pillows ( head and chest slightly raised).

If your symptoms are worsening by physical activity, or after you climb stairs, cardiac condition should be evaluated.

There is need for more information about your symptoms, such issues mentioned above, relations to food, from how long do you suffer them, any other medical issues or past medical history, in order to have a more appropriate evaluation of your concerns.
