Question: I had a small cut on my thump and it happened 12 hours back, accidentally when i was stimulating the outer part of the vagina (a massage and or sex worker) some fluid get in contact with the wound, and i was having a burning sensation on my wound. Immediately i washed my hand with soap and water, is there any possibility for getting infected HIV. Please answe

Please find my observations below.

1. It is important to see the cut and I would request you to please upload a picture of your injured thumb in your query.

2. Please reply back telling when you had the cut injury and when you went to sex worker. If the wound took place many hours earlier and there is no active bleeding from the wound at the time your cut was exposed to the vaginal fluid then there is almost nil chances of HIV transmission.

3. HIV transmission takes place only in actively bleeding cut injuries that has happened due to a deep cut wound.

4. If your wound is small one and did not bleed at time of vaginal stimulation then there is no risk of HIV transmission.

Please reply back with picture of cut thumb, time duration between cut injury and exposure to fluid and if the wound was bleeding at that time.

The cut happened at night time, & I saw the cut in the morning. Afternoon i went for a massage (i think she was a commercial sex worker) and It was not bleeding at that time (cut happened 6 to 12 hrs back), but i got a burning sensation like salt water in the wound.

There was no active bleeding that time. Will upload the picture for the same.

I didn't put my fingers inside the vagina, i was using my middle finger on the top outer portion of the vagina. I think my thumb touched the vagina middle/top outer portion.

Thanks for writing back with an update.

1. It is understood that you had the cut injury to your thumb about 15 to 18 hours before the massage incident. The wound usually starts healing after 6 hours in case of a small cut but the wound is still raw. Nerve endings also get cut and when there is contact of the healing wound with a fluid like the vaginal fluid then it can cause slight pain to be felt.

2. Thanks for sending the picture. The chance for HIV infection is negligible through that wound. The risk is less than 1 in 10000 incidents. It is a good thing that you washed your hand with soap and water.

3. Therefore there is 99.99 percent no risk of getting HIV from above incident. For peace of mind you can get HIV test Rapid 4th generation done after one month or 3rd generation ELISA test done at 3 and 6 months. I am sure the test result will be negative for you.
