Question: I have extremely high blood pressure but my ecg, oxygen and everything else is within normal range. I am on 2 different types of bp medication, atenolol and lisinopril, I have severe anxiety, what is wrong with me?

I carefully passed through your question and would explain that extremely high blood pressure could be related to a panic attack.

From the other hand, just an episode of high blood pressure does not indicate uncontrolled high blood pressure (by your actual antihypertensive therapy).

For this reason, I would recommend you to closely monitor your blood pressure frequently during the day (sitting position after 10 minutes of total relaxation) in the next 5 days and write them down.

If your blood pressure values are above 140/90 mmHg most of the time during the next days, they would be indicative for uncontrolled high blood pressure.

In such case further tests would be needed to exclude other possible causes of secondary hypertension:

- a complete blood count
- PCR, sedimentation rate for inflammation
- blood electrolytes
- thyroid hormone levels for thyroid dysfunction
- fasting glucose
- kidney and liver function tests
- aldosterone and cortisol plasma levels for adrenal dysfunction
- urinary metanephrines for pheochromocytoma

Further changes in your therapy may be needed to help you manage better your blood pressure (increase the dose of your actual medicine or add on other antihypertensive drugs like calcium channel blocker).

I would also recommend consulting with your psychiatrist in order to make the proper changes to your antidepressants (raise the dose), to help you manage better anxiety (which is a trigger of high blood pressure).
