Brief Answer:
They are usually not significant

Detailed Answer:
Dear XXXX
Dear I can understand your concern and I am very sorry for your problems.

First thing I would like to inform you that there is no need to worry about enlarge peri aortic Lymph nodes. lymph nodes are normally present in that area and they are not significant in isolation .

Dear I would like to know

1. in which part of aorta it is found and
2.is there any other information mentioned
3.what investigation was done.
4.what is your age and sex.

Now coming to your back pain that is in upper back in rib cage region and constant pain one half of abdomen..

As you have mentioned very well defined areas in which you feel pain that originate in back and radiate to front and is postural, that is get aggravated by different postures .

On the basis of information it is very sure that it's musculoskeletal pain or pain caused by any nerve root compression in your back.

Most commonly that is caused by

1.abnormal sitting posture
2 . osteoporosis in which bone get softened because of decreased mineralisation specially in old age.
3. Osteoarthritis it's simply wear and tear of bone in weight bearing joints that specially affects bones of spine.

apart from this there are many other causes that we can rule out only by complete history and examination .

If needed MRI thoracic lumbar spine can be done to look for bone, muscles and nerves etc.
so in my opinion you consult to your physician and discuss these issues with him.

hope this will help you.
if you have any other concerns please feel free to message me .
