Question: i am diabetic since last 5 years presently using glimepiride and metformin hcl tab (glimp-m2) and my present glycemic index is 5.8. i am aged 65 years. wt.78 kg, ht 5.4. i have knee joint pains and also suffering from psoriasis patch on my rt. leg below knee and above ankle at present. earlier i had it on my rt. and lft. feet also but it disappeared with chemotherapy i.e. tab. mithotrixate once a week for 10 weeks but forced to discontinue as my blood sugars went uncontrolled i.e. to 400+mg% at that time, leaving light scars. what should i do now.

Based on clinical history and query, my opinion is as follows:

1. Sugar control is essential.

2. For psoriasis, local therapy is advisable. You can try UV light exposure, sunlight exposure, short term corticosteroid topically, salicylic acid or even vit D. All are local and could be helpful. You need to discuss with your doctor, which will suit you.

3. If it gets extensive, you can go for TNF alpha inhibitors therapy like etanercept, etc. It could cause hypoglycemia. Medication needs to be reduced for diabetes. However, increased risk of infection could be present.
4. For joints pain, possible osteoarthritis. Advise physiotherapy and passive exercises along with knee supports.

I would advice local therapy. Methotrexate not advisable.
