Suggest Treatment To Repair Labrum Tears

Question: Hello. I am 49 y.o. female. after traumatic fall and then I had to ambulate for several hours at the end of May this year (2014), I was Dx with hip labrum tear (and I had a significant seroma on hip and buttock which is resolving) per MRI. Any pelvic or hip Fx's were r/o via many xrays and bone scans. Per a ortho physician recommendation, I started P.T. and could not tolerate it. Then, after a physiatrist eval it was decided I should have sx. The ortho physician I saw is great but stated he lacked experience with labrum tear repairs. I smoke 1/2 ppd or less. I do understand the labrum is avascular and smoking is terrible habit. I am now on a nicotine patch and am trying very hard not to smoke at all. The Dr who specializes in the sx I need will not perform my sx unless I totally stop smoking for 30 days and not wear nicotine patch either. I can not walk more than 5 minutes at a time on even surfaces only, or WB to sit properly, I have an awful limp now and can not bend over. It has been over 5 months! I am looking for a surgeon here in Michigan that I could see before January due to ins OOP and I just can hardly take this anymore. There must be surgeons that will repair labrum tears for a light smoker. Can anyone help me/advise me. My overall health is good and have an avg wt. Thank You for any time and consideration to my concerns.

Detailed Answer:
Dear Madam,

Thank you for posting your query on HCM.

Well going through your entire problem I would like to tell you that Smoking is not a contraindication for surgery but your surgeon is saying so because it may cause delay in healing .

Since your surgeon is adamant over this fact then there are two options , either you stop smoking completely or change the surgeon . The second one is a bad option if the surgeon you are under treatment is good and by how much he is concerned for you and your recovery I think he must be a good .

You should give up smoking because labrum tear repair needs a good surgical hand so you should comply with what your treating doctor is saying . You can try out on bupropion or nicotine chewing gums for deaddiction.

Take care.

Dr. Shruti
