Brief Answer:
Surgery is not immediately needed.

Detailed Answer:
Hello,
Thanks for posting your query.
Your brother in law is having a disc prolapse with mild stenosis of the foramina from which nerves come out with minimal nerve compression. Regarding the need of surgery, it depends on his symptoms and how far he is being relieved after taking the proper medicines.
In this case surgery is not an emergency. If he can lead a normal life with the help of medicines then the surgery can be avoided for a while. The disease process will not be reversed with any medicines but the symptoms can be taken care of.
These are due to degenerative changes in the spine. Even after the surgery the further degeneration cannot be stopped. The main treatment of such problem is bed rest along with pain killers and muscle relaxants. Muscle relaxants can help with his symptoms if used in appropriate dosage in combination with a potent analgesic. You can get these prescribed from his orthopedician.
Local analgesic gels and hot fomentation may provide a short term relief. He can use analgesic gels like Volini gel. Exercise is not advisable in the times of acute pain.
Using an LS contour belt of appropriate size might be helpful in providing support to back muscles.
Physiotherapy also plays a major role in treating such conditions.
I hope this answers your query.
