Question: I have a question on risk of Sti from hand Job and anus massage
I had a massage last Friday in a Japanese massage place . She started massaging my thighs , ass ( also teasing with nails) , then cont with a handjob with lubricant which I stopped after a while. She spammed me a bit and my balls.
She scratched me a bit with nails not on penis but on ass.
Now 4-5 days later I find two small ulcrers/wounds in my anal area. ( should note that I sometimes in past do get small wounds in anal area when going to toilet to often , blood on paper) ..
What is risk it is syphils or other Sti..
Very worried now and feel guilty as hell as my newly married wife is pregnant

I am dr.kakkar(dermatologist and venereologist). I have gone through your query and i have reviewed the images.

Thre is no risk of transmission of STIs through hands, nails or a handjob.

STIs are only transmitted from one partner to another only through direct sexual contact i.e vaginal, anal or oral sex.

Thank you for your answer and advice.

I had no direct sexual contact i.e. Vaginal, anal or oral. As wrote she massaged me ( penis, ass, anal (only outside area not inside) , scratched (teased) abit with nails on ass and anal area ( outside ) , spanked my ass and balls a bit.
I only touched her waist and breast outside bra.

Follow up questions
- I did not see if she (or he , not 100% sure was not a shemale, did never se her/him naked or never touched outside/inside panties) touched her genial area but how about if she did and had her genital fluids on her hand ?
I don't belive this was the case but can Sti spread this way ?

- second I did not see / look closely on her hands if had any sore / chancres on hands but if the case could then syphilis have spread to me ?

- can the sores on my ass ( pictures ) be syphilis chancre/ sores from the primary (first) stage of syphilis symptoms. Or what does the doctor think they might be ?

- finally when can I go and be tested for syphilis ? Meaning how long do I need to wait until some accuracy in determine to not have syphilis ? ( I know that for HIV it is 3 month, but less worried about HIV than syphils)

Unlikely to be an STD; Seems to be a result of scratching

Hi.

I apologize for delay in my reply.
The nature of your contact does not put you at increased risk of STIs.

Genital touch or rubbing are absolutely safe. As far as transmission through contaminated fingers is concerned, it is not a usual route.

Moreover, STI causing microbes do not survive outside the body for long and die quickly on drying.

Extra genital chancre on hands is very uncommon and moreover the skin lesions in image do not look like chancres at all.

If you still want to test for syphilis you may take a VDRL test, today and then repeat another test after 12 weeks for a conclusive result.

I did go to a clinic yesterday and made rabid test for syphilis, hiv and hepatit B -all negative as expected , and waiting for answer on clamydia and gonnoree.

Doctor also examined the lesions and he also said/judged that not chancres . He also told me as you said very very unlikely any StI /STD from my encounter . He said no risk.

I also asked about scabies and crablice as got some itichong. He Said he did not see but did not examin for it either.
I have a sensation of itching over my whole body which has increased last coulpke of days especially night time. Feels like somethings on my skin but cannot see anything. No bites at least , maybe a redness and feel a bit dry too.
What can this be ?
Can it be scabies or anything else related to the encounter ?

Xesrosis with itching is the likely possibility

Hi

Scabies is not an STI and gets transmitted by close skin to skin contact. Scabies usually presents with itchy skin lesions at body sites like abdomen, thighs, arms, hands etc Since you fo not have lesions at characteristic body sites elsewhere i dont think it is scabies.
Xerosis or dryness can also lead to scratches or excoriations and this could be the likely diagnosis in your case. I suggest you to use a moisturizer for dry skin and take a tablet of cetrizine 10 mg once daily in evening for symptomatic relief from itching.

Dear Dr. Kakkar S,

As well as pictures of some rash / bites that I found on my arm today.

I took some allergic medic ( loratadin) that I had to stop itching sensation , helped partly as well as after shower and stay dry and cold is helping take down the itching sensation . ( warm and humid here innjapan at the moment to sweating a lot)

Should maybe add that the that the rash / bites on my arm shown in picture is not itching , nor is the same on hands.

The itching is not on particular spots but more overall feeling and tends/feels like moving , can be on abdominal , thighs , legs , arms , back , head , face etc..

hi

You may continuw with loratadine for a few days.
Topically you may apply a topical steroid+antibiotic combination cream/ ointment (e.g either mometasone furoate+ fusidic acid cream Or Fluticasone propionate + mupirocin ointment), 2x daily at the site of rash, however i dont feel that the rashes are due to scabies.

I will continue with loratadin a few days and look for a cream to use.

Last night the itching increased and had hard to seep even I taken the tablet before goi g to bed
I also added some new pictures of the bite/rash on my arm.

If not due to scabies can any STI give these symptoms ?

No. STIs dont usually present with generalized itching.
I feel that you have eczematous dermatitis on legs which is also due to dry skin. I can see 2 coin sized patches on leg. I suggest you to use a topical steroid cream for it as discussed earlier.
Noctural itching is typical of scabies though not diagnostic of scabies. Therefore I feel it would not be a bad idea to take empirical treatment for scabies as well. A single dose of Ivermectol 12mg tablet would be enough.

I don't know how to get Ivermectol 12mg tablet in Japan but will investigate.

I found some some new spots/bites/rash on leg , arm and hand see pictures I attached.
What's your judgement on these ?

Hi.

If you are getting new spots then you should definitely treat for scabies.
If you can get ivermectol 12mg tablet, well and fine.
An alternative to ivermectol is single, whole body application (sparing face) of 5% permethrin cream, left overnight for at least 8 hours and bathing next day.
Apart from that you should continue with application of steroid cream for the spots and also take a tablet of loratadine
