Question: Hello,
I was admitted to hospital on 24sep2016 because of stomach pain and my lft report which showed sgpt 4200, sgot 2600, alp 413,bilirubin direct 4,indirect 0.9. After furthur testing I was diagnosed with hepatitis A and came back negative for B and C and also had developed jaundice with tiredness. On 26sep2016 my lft was sgpt 2390,sgot 1108,alp 238,direct bilirubin 4.3,indirect bilirubin 1.7 and had upper ultrasound which showed mildly enlarged spleen and contracted gallbladder.I was discharged on 26sep16.I was advised normal diet(but I am not taking any oil or fat) with medicines rablet and zevit. 29sep16 my lft was sgpt 1962,sgot 718,alp 299,direct bilirubin 7.5,indirect bilirubin 5.4.My gastroenterologist put me on ursocol450 twice daily. On 6oct16 my lft was sgpt 460,sgot 106,alp,155,direct bilirubin 19.8 mg/dl, indirect bilirubin 9.2mg/dl. Rest total protein,albumin,ggtp within range.I also had liver elastography done which showed avg. liver stiffness 6.65 kpa(negative for fibrosis) with remarks "Liver is normal in size, shape and shows early coarsening of echotexture.No focal echo variant lesion is noted.There is no IHBR dilation. Gallbladder is partially distended. The walls are thickened and edematous.
After this my gastroenterologist put me on Ursocol 450 thrice a day and atarax once along with rablet twice and zevit once.
Till now I have increased my water intake and As long as I keep it high my urine looks more-or-less normal but still my stools are pale colored although some color has returned but can't be sure. I am still taking normal diet with no oil or fat and taking apples,oranges,pomegranate and coconut water and also loads of water intake. As on today my symptoms are tiredness, jaundice,little dark urine,pale stools and mainly INTENSE itching which is preventing me restful sleep at night although compensating by sleeping in day. My main concern is this itching which is becoming intolerable and my total bilirubin level which is 29mg/dl which I have never heard except newborns.My next lft with prothrombin time test is tomorrow and next gastroenterologist visit is on 13oct16
Please guide me what to do.

Hello,
1) First of all you need to be hospitalized for such high bilirubin level. Although hospitalization is only for observation, it is not advisable to take oral medicines to protect/supplement liver function.
2) Yes itching is due to high bilirubin level which deposit in layers of skin and produce itching.
3) In case of severe itching, anti-histaminic group of medications might help to reduce itching.
